For details of your service history, you need to fill out and post the Statement of Service request  form. To access the form open the Datacom link which will take you to the forms.  You need form ESP9.

Please include payment in New Zealand dollars. Credit cards are not accepted.

We are not able to accept emailed requests as, under the Privacy Act 1993, an authorised signature is required to release this information.
